Rewards and Taxes

If you request payments of $600 or more in a calendar year, SurveySpot requires that you complete and submit an IRS Form W-9, a request for Taxpayer Identification Number. The IRS requires that all earnings of $600 or more, per calendar year, are reported on Form 1099-Misc. at year-end, and your taxpayer ID is needed to complete this form.

On the printed form, please write your name, address and Taxpayer Identification Number, then sign and date the form. Mail completed Form W-9 to:
